Logan Physio Terms & Conditions

Welcome to Logan Physio. By booking an appointment, attending our clinic, participating in our services, or using our website, you agree to the following Terms and Conditions.

1. Appointments

Appointments can be made by phone, email, online booking, or in person.

We recommend arriving a few minutes before your scheduled appointment time to allow for any necessary paperwork or updates to your details.

Appointment durations are allocated based on the service booked. Late arrival may reduce the treatment time available.

2. Online Booking Terms

Patients using our online booking system are responsible for ensuring that the information provided during the booking process is accurate and complete.

Online bookings are subject to practitioner availability and confirmation by Logan Physiotherapy.

The clinic reserves the right to amend, reschedule, or cancel appointments where necessary, including where an incorrect appointment type has been selected.

By making an online booking, patients agree to comply with the clinic’s cancellation, attendance, and payment policies.

Patients should contact the clinic if they are unsure which appointment type is most appropriate for their needs.

3. SMS and Email Appointment Reminders

Logan Physiotherapy may send appointment reminders, confirmations, recalls, and other service-related communications via SMS, email, or telephone.

By providing your contact details, you consent to receiving these communications.

While we make reasonable efforts to provide appointment reminders, patients remain responsible for attending scheduled appointments regardless of whether a reminder is received.

Patients should notify the clinic promptly if their contact details change.

Patients may opt out of non-essential marketing communications at any time; however, service-related communications regarding appointments and clinical care may still be sent where necessary.

4. Cancellation and Missed Appointments

We require at least 24 hours’ notice if you need to cancel or reschedule an appointment.

Failure to provide adequate notice may result in a cancellation fee.

Patients who do not attend their appointment without notice may be charged the full consultation fee.

Cancellation fees are payable prior to future appointments being scheduled.

We understand that emergencies and unforeseen circumstances can occur and reserve the right to waive cancellation fees at our discretion.

5. Fees and Payment

Payment is required at the time of consultation unless alternative arrangements have been agreed upon in advance.

We accept cash, EFTPOS, major credit cards, and approved third-party funding arrangements where applicable.

Private health insurance claims may be processed through HICAPS where available.

Any gap payment remains the responsibility of the patient.

WorkCover, DVA, NDIS, Medicare, or other third-party funded services may be billed in accordance with the requirements of the relevant funding body.

Patients remain responsible for any fees not covered by third-party funders.

6. Treatment and Clinical Care

Our practitioners will provide services in accordance with professional standards, clinical guidelines, and their scope of practice.

Healthcare outcomes cannot be guaranteed. Individual responses to treatment vary and depend on numerous factors including diagnosis, medical history, adherence to advice, and lifestyle factors.

Patients are encouraged to provide complete and accurate information regarding their health history, medications, symptoms, and any changes in their condition.

Failure to provide relevant information may affect treatment outcomes.

7. Consent to Treatment

By attending an appointment, patients consent to assessment and treatment by their practitioner.

Treatment options, risks, benefits, and alternatives will be discussed where appropriate.

Patients may withdraw consent or decline any treatment at any time.

8. Children and Minors

Patients under 18 years of age must have a parent, guardian, or authorised representative provide consent for assessment and treatment, except where otherwise permitted by law.

A parent, guardian, or authorised representative may be required to attend appointments where clinically appropriate.

Where a practitioner determines that a minor is capable of understanding the nature and consequences of treatment, consent may be obtained from the minor in accordance with applicable laws and professional standards.

Parents and guardians are responsible for providing accurate and relevant health information relating to the child and for notifying the clinic of any changes to the child’s health status, medications, or circumstances that may affect treatment.

9. Telehealth Services

Where telehealth services are offered, patients acknowledge that:

  • Technology failures may occasionally interrupt consultations.
  • Certain assessments or treatments may be limited in an online environment.
  • Alternative arrangements may be recommended where clinically necessary.

10. Hydrotherapy Participation

Patients participating in hydrotherapy sessions acknowledge that water-based exercise carries inherent risks, including slips, falls, fatigue, and aggravation of existing conditions.

Participants agree to:

  • Follow all instructions provided by clinic staff and facility personnel.
  • Inform their physiotherapist of any changes in their medical condition.
  • Advise staff immediately if they experience pain, dizziness, shortness of breath, illness, or any unusual symptoms.
  • Use hydrotherapy facilities in a safe and responsible manner.

Participation may be restricted or discontinued where a physiotherapist determines that hydrotherapy is not clinically appropriate or safe for the participant.

Patients are responsible for ensuring they meet any venue-specific requirements relating to pool access, hygiene, or safety.

11. Photography and Video Consent

From time to time, Logan Physiotherapy may request permission to take photographs or video recordings for educational, promotional, marketing, website, social media, or clinic communication purposes.

No photographs or videos identifying a patient will be taken or published without the patient’s prior written or verbal consent.

Patients may decline photography or video recording without affecting the care or services they receive.

Where consent has been provided, patients may withdraw consent for future use by contacting the clinic. Withdrawal of consent will not affect material that has already been published or distributed prior to the withdrawal request.
